MEDICAL CASE MANAGEMENT SOLUTIONS
For more than a decade, Chapman & Associates has offered Medical Case Management Solutions for workers whose injuries fall under Workers’ Compensation in Colorado and nationwide. Under the direction of Mindy Chapman, R.N., CCM, our nurse case managers and staff work closely with providers to provide carefully monitored, plan-directed care. Our process integrates evaluation, treatment, medical record review, referral and follow up to ensure the prompt resolution to a claim and or return to work. Our Case Managers diligently follow the progress of the injured worker toward Maximum Medical Improvement and Return to Work.
Chapman & Associates acts as the liaison between the injured employee, the claims adjuster, employer and the providers in the Workers’ Compensation system. Services may include the following:
· Initial Report and Review of Case
· Case Progress Reports
· Coordination of Conferences with Providers, Attorneys and Injured Worker and their family
· Set up and follow post hospital or outpatient care, pain clinics and Physical and Occupational Therapy Programs.
· Obtain appropriate Medical equipment as needed
· Communication between parties involved in the case including the injured worker, his/her family when appropriate, adjuster, attorney, and Case Managers
· Expert Testimony
